Ronald Bush, MD, FACS
The Midwest Vein and Laser Center, Centerville, Ohio
Dr. Bush is one of the nations leading specialists in venous disease. Dr. Bush founded and is the
medical coordinator for Vein Affiliates, a national group of surgeons specializing in venous disease.
He is Board Certified in thoracic surgery and general vascular surgery. He is the author of numerous
scientific papers and holds patents for surgical instruments. For the past 15 years, Dr. Bush has
devoted his practice solely to the treatment of venous disease. Dr. Bush and his staff have performed
more than 6,000 venous surgeries and thermal ablations in the office setting.
Dr. Bush completed medical school and his general surgery residency at Indiana University,
completed a Peripheral Vascular Fellowship from the Baylor Medical Center, and completed a
cardiovascular/cardiothoracic surgery residency from Walter Reed Army Medical Center.
Robert Provenzano, MD, FACP
St. John Hospital & Medical Center, Detroit, MI
Dr. Provenzano completed his internship and residency in internal medicine at St. John Medical
Center, and his nephrology fellowship at Henry Ford Hospital in Detroit, MI. Dr. Provenzano is
currently Chief of the Division of Nephrology, Hypertension & Transplantation, Director of
Nephrology Research, and Director of Acute Dialysis Services at St. John Hospital & Medical Center.
He is also an Associate Clinical Professor of Medicine at Wayne State University School of Medicine.
He is the Chief Operating Officer of St. Clair Specialty Physicians in Detroit, and the Chief Strategy
Officer for DaVita, Inc.

Lawrence Colen, MD
Dr. Colen received his medical degree from Dartmouth Medical School, and continued his training in
general surgery and plastic surgery at the University of California San Francisco. Certified by the
American Board of Surgery and the American Board of Plastic Surgery, Dr. Colen continues to publish
articles in plastic surgery and contributes to the texts of several medical textbooks. In 1994 he was the
first recipient of the National Godina Fellowship Award in Microsurgery. Dr. Colen regularly presents
at symposia for plastic surgeons, while he continues to develop his own expertise in the most recent
and innovative approaches in plastic surgery.
David J. Dexter II, MD

Dr. Dexter completed his medical degree at State University of New York at Syracuse, and continued
his general surgery residency at the University of Maryland Medical Center. His fellowship in
Vascular and Endovascular Surgery is being complete this year at the New York University. Dr.
Dexter also had the exciting opportunity to complete an international vascular surgery fellowship in
Shanghai,China.
